Empowering Communities through Culturally Safe Care Coordination
We are seeking a dedicated Care Coordinator to provide care coordination and wellbeing support to community members living with mild to moderate mental health needs in the New England region.
The ideal candidate will work collaboratively with a multidisciplinary team to improve access to services, build stronger referral pathways, and promote holistic health and wellbeing for Aboriginal and Torres Strait Islander communities.
* Culturally safe care coordination practices that prioritize the needs of Aboriginal and Torres Strait Islander people
* Supporting access to mental health services, general practitioners, and community providers
