Our workplace culture is based on the BEST philosophy, underpinned by our values and customer service charter. This role forms part of our Home Care program, which involves conducting clinical assessments and providing support to clients within the Ballarat and surrounding communities.
About this Role
* Perform in-home clinical assessments, reviews, and care needs for clients.
* Provide clinical advice and support to clients, referring them to relevant services when necessary.
* Collaborate with local allied health professionals to ensure responsive care delivery.
* Engage in health education and promotion activities, including social support groups and carer education.
* Promote a harmonious team approach and implement safe practice measures.
Requirements and Qualifications
* A Bachelor's degree in Nursing or equivalent qualification accompanied by current AHPRA Division 1 registration.
* A minimum of 3 years' experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing.
* Demonstrated teamwork skills, positive outlook, and flexibility in approach.
* Endorsement for Continence Assessments and Prescription (desirable).
* Experience with evaluating and implementing care support plans.
* Familiarity with the My Aged Care portal.
* Proficiency in decision-making, critical analysis, and problem-solving.
This is a full-time position, Monday to Friday during business hours. The office-based role offers flexibility in accommodating organization and client needs.
