About the role This position is responsible for the provision of responsive, person- and family-centred Social Services, to patients in the Persistent Pain program, delivered in partnership with Queensland Health. As a member of the broader multidisciplinary team delivering comprehensive pain services in the Metro North region of Southeast Queensland. The role works within a strength-based and recovery-oriented framework, supporting the broader social and environmental influences on health and wellbeing as well as providing individual and family assessment, management and support. Within this role, you will be required to: Work collaboratively as a member of the multidisciplinary Pain Service team, including both IUIH and MNH employed professionals and support staff Work collaboratively with clients, families and supports, to identify their social health and well-being needs and goals, formulate a plan of social care, and provide assistance to clients to achieve their goals Provide outreach and community-based care, in accordance with the needs of the client Collaborate with the staff from TCPRC and across MATSICHS Pain Services Program, providing a point of contact and provide professional advice, building knowledge and skills in the provision of follow up social care and support for clients accessing the Pain Service, and Provide supervision and / or support to Social Work students – and potentially other students in related fields - undertaking placements with MATSICHS About you Current C Class Drivers Licence (QLD) Current Criminal History Check– willingness to obtain upon commencement Current Working with Children Clearence (QLD Blue Card) Required prior to your commencement Current First Aid and CPR Certificates – willingness to obtain upon commencement Relevant degree in Social Work or Behavioural Sciences What we offer At IUIH we recognise that we operate in a diverse community and welcome and encourage applications from all ages and genders, Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, the LGBTIQ community, veterans, refugees, and people with disability.
