Are you a passionate and experienced Care Manager in Aged Care and looking for your next challenge? We have an incredible opportunity for a Care Manager to lead our 45-bed aged care facility a short distance from Young NSW. This facility boasts a fantastic culture, excellent operational performance, and a strong reputation for delivering high-quality care.
About the Role: You will play a key leadership role in ensuring the delivery of safe, high-quality, resident-centred care. Reporting to the Residential Service Manager, you will guide and support our care team, ensuring compliance with the Aged Care Quality Standards and Apollo Care's model of care.
As the Care Manager, you will be responsible for:
* Lead and support the care team to deliver holistic, person-centred care.
* Oversee care planning, clinical governance, and resident assessments.
* Ensure compliance with regulatory requirements, including the Aged Care Act 1997.
* Promote resident choice, family engagement, and a strong sense of home.
* Drive continuous improvement in quality, safety, and resident experience.
About You:
* Current registration with AHPRA as a Registered Nurse.
* Minimum 5 years' experience as a Clinical Nurse, Care Coordinator, or Care Manager (with at least 1 year in aged care).
* Strong leadership and team management skills.
* Excellent knowledge of the Aged Care Quality Standards and clinical governance.
* A genuine commitment to supporting residents to live their way with dignity and grace.
* Strong verbal and written communication skills
