OnTrack Tasmania is seeking a Participant Intake and Engagement Case Manager to oversee person-centred, high-quality services for people with disabilities. This role involves managing intake and engagement for independent, short-term, and medium-term accommodation while providing intensive, recovery-focused support for individuals with complex needs, such as those with a history of homelessness, addiction, and dual disabilities.
Key responsibilities include:
* Coordinating participant intake and engagement
* Monitoring participant wellbeing, progress, and support needs
* Ensuring compliance with NDIS standards, policies, and procedures
* Collaborating with teams to match participants with appropriate support staff
* Maintaining accurate documentation and reporting
* Supporting continuous improvement and maintaining audit readiness
