**Monash Health appointed lead provider for the new Infant, Child Family Health and Wellbeing Hub**
Monash Health's **Infant Child and Family Health and Wellbeing (ICF H&W) Hub** is a new service dedicated to providing therapy that involves all aspects of an infant's or child's life from 0-11 years. As a specialist multidisciplinary service, the **ICF H&W** will provide support to families with children experiencing developmental, behavioural, and emotional challenges with wellbeing and peer support combined with care coordination to help with different kinds of challenges including developmental, behavioural, and mental health concerns.?The **ICF H&W Hub** has funding for community health, mental health, and family support services to enable mental health consultations, assessments, and treatments to infants, children and families in the South-eastern region of Victoria. Designed as a central one-stop location for mental health, health and wellbeing, it will deliver expertise across a range of specialist medical and allied health services, including paediatricians, occupational therapists, psychologists and speech pathologists and offer parenting supports. **The ICF** **H&W Hub** will sit alongside **Early in Life Mental Health Service** **(ELMHS)** - Monash Childrens and the Child and Family Program - Monash Health Community.
**Aboriginal Family Support Worker - Monash Health Community**

As **Aboriginal Support Worker** at **Monash Health Community**, you will provide paediatric Aboriginal clients in the **ICF H&W Hub** with a holistic and culturally appropriate?journey with consideration of the social determinants of health which impact the Aboriginal Community. Working collaboratively with the multi-disciplinary?team in the care of Aboriginal children and their family whilst they are in the Hub, you will assist and support clients and their families to navigate the health system. This service will be a Hub and Spoke model. The Hub will be located at Cranbourne, with spoke sites across the catchment of Dandenong, Casey, and Cardinia and will be part of a newly co-designed service with a Transdisciplinary/ Multidisciplinary service.
Operating within the Community Priorities directorate, you'll work closely and collaboratively with a highly dedicated, experienced and professional multi-disciplinary team from the Child family program and Early in Mental Health Service. You are truly excited to be part of the new **ICF H&W** **Hub** dedicated to providing a range of specialist medical and allied health services to ensure that infants, children, and their families receive the treatment, care and support they need.
**New Part Time (.4 eft) position now available - Located at Cranbourne with travel to spoke sites, as required.**
**What you Bring**
As the Aboriginal Family Support and community development worker, you'll support paediatric Aboriginal clients in the ICF H&W Hub with a holistic and culturally appropriate journey through the hub. To ensure your success in this role, you will have the following demonstrated skills, experience and knowledge:
- Relevant Paediatric qualifications to demonstrate your ability to perform role.
- A relevant case-management qualification or Children's services qualification.
- Identify as an Aboriginal and/or Torres Strait Islander person and demonstrate acceptance by their community and actively participate in Aboriginal/Torres Strait Islander cultural activities
- Understanding of Victorian Aboriginal communities, and ability to build connections with Aboriginal children, families, and care givers.
- Experience advocating and supporting the needs of marginalized and/or underrepresented communities within mainstream systems.
- Dedicated to safe best practice and quality care
- Committed to delivering patient centred care and a patient advocate
- Enthusiastic, compassionate able to work collaboratively across multidisciplinary team
- Current Drivers Licence
