In this role, you will be conducting assessments for clients identified with entry-level needs that can be met through the Commonwealth Home Support Program (CHSP), while also facilitating their access to this program.We are seeking candidates with a minimum Certificate IV in an appropriate Health and/or Human Services field or extensive relevant experience. Working in a supportive multidisciplinary team, this role requires skills in time management, problem-solving, working under minimal supervision, and advanced computer skills/knowledge.If qualifications are a requirement for this position, please attach a copy in support of your application.
