Role Summary
The Clinical Nurse Specialist Grade 2 (CNS2) plays a critical leadership role within the Care Navigation and Care Coordination team. The CNS2 provides expert clinical oversight, specialist decision-making, and advanced coordination to support timely and safe patient transitions across the care continuum. This position guides frontline clinicians, leads complex care navigation, and supports the resolution of barriers that would otherwise delay discharge, escalate avoidable presentations, or extend patient length of stay. The CNS2 serves as the senior clinical anchor for the team, ensuring high-quality assessments, prioritised workflows, and strengthened relationships with patients, primary care, and community services.
Responsibilities
* Provide leadership capacity to enhance team ability to deliver key service outcomes and ensure safe, reliable, and responsive care.
* Directly contribute to improved patient experience, reduced system pressure, and overall service performance across P&CH.
* Lead complex care navigation and coordinate care across acute, primary, community, and social service providers.
* Facilitate prioritised workflows and mentor frontline clinicians in a culturally responsive, person-centred approach.
Are You the Right Fit?
The ideal candidate has advanced clinical skills and a strong foundation in chronic and complex care management, supported by relevant postgraduate qualifications.
* At least three years of post-registration experience with the ability to exercise extended autonomy, critical thinking, and sound professional judgement in complex clinical situations.
* Experience mentoring and coaching nursing staff, contributing to quality improvement, and modelling person-centred, culturally responsive care.
* Strong communication and interpersonal skills, proficiency with electronic health systems, and the ability to work independently and within multidisciplinary teams.
* Capability to deliver comprehensive assessments, partner with vulnerable clients and families, and coordinate care across multiple sectors.
* Adaptable, proactive, and committed to supporting clients to stay well in the community.
Selection Criteria
* Authority to Practice as a Registered Nurse with AHPRA, relevant post-registration qualification, and at least 3 years full-time equivalent clinical experience in the relevant area.
* Overview of education, training, and clinically relevant experience for the role.
* Demonstrated advanced clinical assessment skills for clients with chronic/complex health needs and ability to exercise extended autonomy in community or integrated care settings.
* Proven ability to engage with vulnerable clients, families, carers, and multidisciplinary teams to deliver safe, compassionate, culturally responsive, person-centred care.
* Ability to mentor, coach, and support nursing staff, contribute to evidence-based practice, and participate in quality improvement and service development.
* Strong critical thinking, problem-solving, and organisational skills; ability to work independently and navigate complex care pathways with proficiency in electronic health records and data systems.
* Current Australian C-Class driver's license (including P1 or P2). If licensed in another state, must obtain NSW C class (P1 or P2) within three months of appointment. Learner licenses not permitted.
