An exciting opportunity is currently available for an experienced Clinical Nurse or Psychologist or Occupational Therapist or Social Worker - Senior (Aged Care Assessor Holiday Reliever) to join the Aged Care Assessment Team, Integrated Care Services at the Bundaberg, Hervey Bay or Maryborough Hospitals within the Wide Bay Hospital and Health Service. The Wide Bay area is arguably one of the most beautiful regions of Queensland. The Health Service stretches from Agnes Water in the North to Maryborough in the South and provides acute care services to a population of over 225,000 residents including several rural communities.

Your Opportunity:

* The Aged Care Assessor within the Wide Bay Aged Care Assessment Team is required to work within a multidisciplinary team and provide clinical assessment services to clients through the use of high level clinical skills, knowledge and experience and highly effective communication. The position works within an integrated hospital/community Aged Care Service across the Service Area in order to assess clients to access appropriate support services and/or residential care.
* This position reports professionally to the relevant Professional Lead and operationally to the Aged Care Assessment Team Leader, Integrated Care, Wide Bay Hospital and Health Service.

What we're looking for:

* Demonstrated comprehensive knowledge of contemporary nursing practice within an Aged Care Assessment Team and the ability to apply advanced professional judgement, and critical thinking to negotiate complex clinical requirements and to appropriately manage patient-centred care requirements.
* High level knowledge, expertise and skill in Occupational Therapy / Social Work / Physiotherapy / Aged Care assessment of frail older people seeking support to make choices about care arrangements who are living with complex psychosocial situations, as well as interventions to assist client to access the most appropriate care and services.
* Understanding of the legislation and policies relevant to the work in the Aged Care Assessment Team particularly in relation to the Aged Care Act 1997 and Aged Care Principles 1997.
* Demonstrated ability to provide high level clinical advice and communicate effectively with clients, families, referrers, other Health Professionals/ and external agencies, and effectively contribute to professional and multidisciplinary teams.
* Demonstrated experience in clinical leadership including the provision of supervision, support and guidance to less experienced Team Members and undergraduate students.

Preferred Qualifications:

* Mandatory possession of a nursing qualification from a registered tertiary institution or completion of an approved hospital training program and Registration with the Australian Health Practitioner Regulation Agency with a current annual practicing certificate is essential.
• Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
* Possession of context-specific postgraduate qualification in nursing is highly desirable.
* For Psychologists:
Mandatory possession of a qualification from a registered tertiary institution as a Psychologist and Registration with the Australian Health Practitioner Regulation Agency with a current annual practicing certificate is essential.
* For Occupational Therapists:
Mandatory possession of a qualification from a registered tertiary institution as an Occupational Therapist and Registration with the Australian Health Practitioner Regulation Agency with a current annual practicing certificate is essential.
* For Social Workers:
Mandatory possession of a recognised degree in Social Work, and eligibility for membership with the Australian Association of Social Workers (AASW). Eligibility is dependent on the candidate having completed a University level Social Work degree accredited by the AASW and that the candidate does not have any other outstanding professional competency or ethical issues or complaints known to the AASW which may preclude membership.
* Overseas qualified Social Work candidates require a "certification of eligibility" letter from the AASW to confirm their eligibility for membership. Overseas qualified applicants should include such evidence with their application to be considered for short-listing in the selection process.
* Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
* Aged Care:
Potential applicants are advised that the Aged Care Act 1997 requires Queensland Health employees and volunteers of aged care services to have a current National Police Certificate. Queensland Health will facilitate the applicants obtaining the above check.
* Applicants in Aged Care
are also advised that appointment to this position is subject to a Banning Order Register check, which will be conducted by the line manager to verify your eligibility to work in the aged care sector.

Our offer:
You will receive a competitive remuneration package value up to $157,360 p.a., comprising salary between $128,169 - $137,918 p.a. (HP4) or competitive remuneration package value up to $135,856 p.a., comprising salary between $111,198 - $119,071 p.a. (Nurse Grade 6 (1)), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)/HP4) (Permanent Full Time, 76 hours p.f.)
