We are seeking a highly skilled and experienced Clinical Care Coordinator to join our team in Hahndorf, SA. As a Clinical Care Coordinator, you will play a crucial role in ensuring the highest quality of clinical care is provided to our residents.

Main Responsibilities:

* Provide leadership and clinical expertise to our care service teams across the facility.
">
* Work closely with the Clinical Care Manager to lead carer teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
">
* Manage all aspects of assessment, documentation, and submission of AN-ACC requirements within the facility.
">
* Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
">
* Coordinate assessment, care planning, and review processes within the facility.
">
* Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
">
* Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records, and staff time recording).
">
* Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
">
* Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
">

About You:
">
To be successful in this role, you will need to have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them.
">
">
* Minimum 1 year experience as a Care Coordinator or similar leadership position.
">
* Must be available to work across Monday to Friday AM or PM shifts.
">
* Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
">
* Postgraduate qualifications in aged care would be advantageous but not essential.
">
* Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
">
* Engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
">
* High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
">
* A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
">
* Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
">
* Have a valid NDIS Workers Screening Clearance (or ability to apply via relevant authorities).
">
