Our Journey
The Proactive Support and Management team are a warm and inviting team who work collaboratively with our interdisciplinary colleagues ensuring the best possible care to our clients. Working within this wider multidisciplinary team, this role will see you providing quality evidence based care to our clients aged 18-65 years old who have chronic health conditions. The Proactive Support and Management team works with community dwelling and this role offers the opportunity to work with clients who are of varying backgrounds including a refugee background.
As a Physiotherapist you can make a significant difference to our diverse and vulnerable clients by enhancing their physical capabilities and supporting clients on their health journey. We offer robust supervision with opportunities for mentorship and development programs to grow and expand in your career.
A Day in the Life
This is a unique and privileged opportunity to make a positive impact on those who need it most in our community. You will work with clients from various backgrounds Including refugees and CALD communities, to offer support, clinical expertise and make a difference to their quality of life. The role involves independent assessment, identification of primary concerns and developing strategies for management of clinical care to clients with Chronic and complex health conditions.
You will provide care in across the Monash Health Catchment. You will educate your clients so they can feel empowered to improve their quality of life. This is a part time ongoing opportunity working Monday to Friday.

What you need:
7. Bachelor of Physiotherapy
8. Current registration with AHPRA
9. Current Drivers Licence
