Job Title: Connected Care Coordinator
The Department of Justice and Community Safety is seeking a Connected Care Co-ordinator to provide critical support at Western Plains Correctional Centre.
This role involves managing and co-ordinating the care of prisoners with multiple and complex needs, ensuring appropriate services and supports are delivered.
* Care Coordination: Develop and implement individualised care plans to meet the diverse needs of prisoners.
* Multi-Disciplinary Collaboration: Lead case-conference meetings to ensure effective working relationships between internal and external service providers.
* Service Delivery: Coordinate and monitor service delivery into specialist units, ensuring high-quality care is provided.
To be successful in this role, you will have demonstrated experience in case management and case co-ordination, including assessing complex needs, developing support plans, and co-ordinating service delivery. You will also have knowledge of community resources and services, and experience in connecting people with services.
