Carer Respite Coordinator / Community Options
Permanent Part Time – 72 hours per fortnight
$*****-$***** per hour + salary packaging
About The Role
Support carers and older community members to live well and independently at home.
In this rewarding role, you'll coordinate services, connect people with supports, and make a meaningful difference in their lives.
Assess and coordinate supports for CHSP and SCP clients
Liaise with carers, subcontractors, and allied health providers
Promote wellbeing and independence for carers and clients
Facilitate group activities and community connections
Key Responsibilities
Assess and review client and carer needs in partnership with them
Coordinate timely referrals and service delivery within program guidelines
Maintain accurate client records, documentation, and reporting
Support carers through advocacy, information, and conflict resolution
Communicate effectively with subcontractors and service providers
Participate in team duties and facilitate group events
Skills & Experience
Qualification in welfare, social work, nursing, or allied health
Advanced communication and interpersonal skills
Strong organisational and time management ability
Excellent computer and reporting skills
Current Victorian driver's licence
Desirable Experience in community-based services, carer engagement, and multidisciplinary teamwork.
