About The Opportunity
The Community Older Persons Intervention and Liaison Outreach Team (COPILOT) is a dynamic and exciting service that provides rapid access to multi-disciplinary geriatric assessment (medical, nursing, and allied health) for older people living in the community setting. Referral enables care to be implemented in the patient's home environment, avoiding unnecessary transfer to hospital, and leading to improved patient outcomes.
What You'll Be Doing
The Senior Physiotherapist is responsible for providing high quality physiotherapy services. This includes service development and evaluation and advanced reasoning and clinical practice skills in the management of clients of a novel or complex nature. This position is responsible for the supervision, teaching and training of Level 1-2 Physiotherapists, AHAs and students. The position will be responsible for providing care to keep consumers well in the community through facilitating early, safe discharges from hospital and reducing unnecessary admissions to hospital.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

How To Apply

* Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
* Relevant clinical experience including demonstrated advanced practice skills and management of a complex physiotherapy caseload including within community care settings and with people with chronic diseases.
* Demonstrated knowledge and experience in quality improvement, research and service development activities.
* Demonstrated effective time management skills and ability to work independently and as part of a multidisciplinary team.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
