- Why this role matters
- The Hobart Street to Home program operates within the Tasmanian Division of The Salvation Army. The program provides accommodation and flexible support to people who are rough or street sleeping, and at risk of primary homelessness
- The role you could play
- We are currently seeking a values-driven and proactive Assertive Outreach Worker to work with people (out of office) who are experiencing primary homeless, rough or street sleeping, and at risk of primary homelessness, to develop and implement case plans that will facilitate their transition to supported accommodation or independent living. You will be working as part of a larger team and alongside our existing Street to Home Outreach worker.
- You will successfully
- Establishing effective relationships with rough and street sleepers, and people at risk of primary homelessness to encourage self-determination and social and economic participation.
- Providing assertive outreach and support to clients in accordance with TSA homelessness case management principles, models and standards of practice.
- Providing ongoing assessment of clients' housing and health issues, needs and circumstances from initial intake to exit.
- Developing individual case plans in collaboration with clients that includes a range of interventions that address the needs and goals of clients.
- Implementation of case plans with active client participation.
- Assisting clients to access appropriate support services and establish community supports, and where needed ensuring clients are assisted through advocacy with service providers.
- This is a full time position, 38 hours per week fixed term for 12 months.
- You will have
- Experience and expertise in the areas of Primary Homelessness, Poverty, Social isolation, Domestic Violence, Mental Health, general health, people exiting prison, Disabilities, Youth, Legal issues and AoD
- Tertiary qualification in social work, community welfare, community development or a related field, a degree qualification is preferred
- Highly developed knowledge of primary homelessness and the social housing service system, in particular the homeless sector
- Demonstrated experience and expertise in working within a psychosocial framework with primary homeless clients who may have complex mental health, health and. or addiction issues, and intellectual and physical disabilities
- Highly developed case management, case work and case planning skills
- Ability to work flexibly, responsively and collaboratively within a diverse team in a dynamic and challenging environment
- High-level oral and written communication skills and well developed interpersonal and computer skills
- Ability to provide assertive outreach to the target group e.g. rough sleepers and couch surfers
- Well-developed homelessness sector knowledge, networking and linkage skills
- Knowledge of models of homelessness intervention including structural and individual explanations of homelessness and agency
- A current Employment Working with Vulnerable People check
- A current Tasmanian Drivers Licence is required
