Southern Cross Care QLD has an exciting opportunity for a proactive and self-directed part-time Care Partner to join the Community Team working from our Community Care office in Raceview.

About the role

As a Care Partner for the Ipswich area, you will work collaboratively with our elderly clients, who live in the community to engage in services that enable them to remain living in their home.

In this dynamic role, you’ll manage a case load of Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) clients, driving towards exceptional care outcomes. Your expertise will ensure that each client’s needs are met with top-quality service and support. You’ll be the key player in coordinating care strategies and using your in-depth knowledge and skills to make a real difference in our clients’ lives. Key Responsibilities Will Include

* Assessing the needs and preferences of each client in a caseload
* Developing personalised care plans in collaboration with clients and their families
* Coordinating the delivery of services such as personal care, meal preparation, cleaning, and transportation
* Monitoring the implementation of care plans and adjusting as needed
* Ensure CHSP services are provided in accordance with CHSP guidelines
* Advocating for clients' rights and preferences
* Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively
* Providing information and support to clients and their families regarding available services and resources
* Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care

What will you bring to the role?

* Experience in a similar role within Aged Care or Disability Services Sector
* Demonstrated knowledge and experience in delivering services under Home Care Package Program
* Demonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP)
* Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislation
* Ability to manage time efficiently and prioritise responsibilities
* Excellent communication skills, ability to communicate clearly and effectively with team members and clients
* Exceptional interpersonal skills and the ability to relate to a variety of people

Southern Cross Care Queensland (SCCQ) is a Catholic not-for-profit organisation established by the Knights of the Southern Cross, more than 40 years ago. They saw a need to provide compassionate care and inclusive service to some of the most vulnerable, the ageing population, in the community.

Since then, SCCQ has grown to employ over 1700 staff caring for and supporting more than 2000 people across 17 residential aged care homes, 8 retirement villages, 28 social houses, the National Disability and Insurance Scheme (NDIS) and home care services in metropolitan and regional Queensland, New South Wales and Victoria.
