Case Manager Position
This role involves taking a holistic approach to supporting clients with diverse needs and issues.
Key Responsibilities:
* Conduct comprehensive assessments of clients' situations, identifying strengths, weaknesses, and needs.
* Develop client-centered care plans outlining specific goals and interventions to address client requirements.
* Connect clients with relevant resources and services, ensuring timely access to necessary support.
* Advocate for clients with other stakeholders, promoting the best interests of the individual.
* Regularly monitor, evaluate, and document client progress, making adjustments to care plans as needed.
Essential Requirements:
* Proven experience in case management using a holistic and client-centered approach.
* Extensive knowledge of government-funded programs and contractual requirements.
* Ability to build and maintain effective relationships with stakeholders, including community organizations and service providers.
* Strong understanding of organizational policies and operations.
* Relevant industry qualifications or experience.
