About The Position
The Aboriginal Care Navigator, Medicare Mental Health Centre Campbelltown, works collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care. This position delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time. This position also works collaboratively with the other members of the Medicare Mental Health Centre Campbelltown team and external service providers to maximise care outcomes for Aboriginal consumers.
This position is a full-time opportunity. Full-time employees can opt to enjoy a rostered day off each month.
The service will operate seven days a week and candidates should be flexible and available to work on a rostered basis, including on weekends and outside of normal business hours, as needed.
We Are Looking For Someone With
* Identify as an Aboriginal and/or Torres Strait Islander and be an active member within the Aboriginal communities of Campbelltown and surrounding areas.
* A tertiary qualification in a relevant discipline, and/or extensive experience in Aboriginal health, AOD, mental health, community services or social sciences.
* Demonstrated ability to engage with Aboriginal consumers and their families or carers to provide care navigation and referral support.
* Demonstrated understanding of supports and services available in the local and regional communities.
