Primary objective

To coordinate and provide an opportunity for clients to reach restorative goals by improving physical, psychosocial and or cognitive functioning.

This program aims to reduce the risk of hospital readmission and premature entry into residential aged care by utilising a multi-disciplinary team

approach. This position is part of the Aged Care Unit which provide a number of services designed to meet the needs of our ageing population

delivering a person centred, wellness and reablement approach.

Context statement

The Aged Care Unit consists of the Aged Care Assessment Service, Short Term Restorative Care and Transition Care, Community Frailty Service

and Memory Service and Psychogeriatric Services.

Key duties and responsibilities

1. Establish relationships with consumers to develop therapeutic goal orientated care plans.

2. Provide coherent and integrated case management that enables care recipients to meet their goals.

3. Consult with internal and external service providers in the planned delivery of required services; evaluate the progress and effectiveness of

the service plans in meeting the agreed consumer outcomes.

4. Actively promote self-management and self-sufficiency, supporting consumers to make the most of their capacity so that they will be able to

continue to participate to their full potential.

5. Implement collaborative service delivery model that delivers seamless care through the purchase of new services and or expand on existing

services.

1. Develop and acquit brokerage funding in accordance with individualised funding agreements.
2. Liaise with service providers and relevant health professionals to improve consumer health literacy and access to the broad range of health

and support services available in the community.

8. Follow defines service quality standards, work health and safety policies and procedures relating to the work being undertaken in order to

ensure high quality, safe services and workplaces.

Selection criteria

Essential

1. Experience working with consumers and their families within the Flexible Care Service model. Professionally qualified Allied Health

Professional or Nurse registered with the relevant Board of Australia.

2. Demonstrate knowledge and experience of community aged care including the theoretical framework, practice standards and ethics, My Aged

Care, Aged Care Quality Standard, and Continuous Quality Improvement

3. Well-developed consultative and liaison skills with the ability to be innovative, flexible and responsive to achieve outcomes in line with the

consumers assessed needs.

4. Established skills in the delivery of an integrated case management model, proven ability in goal setting, and achieving set timeframes.

1. Well-developed communication and information technology skills.

2. Proven experience in managing financial resources.

3. Proven ability to work independently and participate effectively in a multidisciplinary team.

4. Awareness and understanding of the complexities of diverse cultures and their relationship to health and wellbeing outcomes for those

accessing the Transition Care/Short Term Restorative Care Program

9. Hold or ability to obtain an NT driver's license.

Desirable

1. Qualifications in Community Care. Post Graduate studies in Aged Care.
