We’re expanding and are looking for a dedicated Nurse to join our Access Aged Care team in the Ingham Shire.

What You’ll Do
- Work alongside RACF care teams as the on-site Access Aged Care nurse.
- Facilitate GP rounds (both in-person and telehealth) to ensure smooth, timely care.
- Prepare, organise, and triage referrals to maximise efficiency and outcomes.
- Conduct bedside assessments and clinical reviews.
- Act as the communication bridge between RACFs, GPs, and families.
- Deliver excellent customer service, with empathy and professionalism at every interaction
- Build strong relationships with residents, families and facility partners.
- Use your clinical knowledge and IT skills to ensure quality outcomes.

What You’ll Bring
- Diploma of Nursing and AHPRA registration.
- Aged Care experience (desirable).
- Robust organisational, problem-solving and communication skills.
- Confidence with IT and documenting resident needs.
- A driver’s licence and reliable vehicle.
