Your new role: In this specialised clinical position, you will provide advanced nursing assessment and kidney care to patients requiring Peritoneal Dialysis. Key responsibilities include: * Delivering advanced assessment, planning, implementation and evaluation of Peritoneal Dialysis care for patients with complex kidney conditions. * Supporting the coordination of care with strong communication, safe documentation, and proficient EMR practice.
