Support Mental Health in a Diverse Community

This role offers a unique opportunity to provide assertive case management, education and support to young people at risk of or experiencing mental illness.
\
The Youth Enhanced Service (YES) aims to increase service access to improve the chances of recovery and longer term social and emotional health and wellbeing. This is achieved by engaging with young people, their family and carers to improve their knowledge and understanding of the impact of mental illness.

Key Responsibilities:
\

1. Implement and review care plans
\
2. Assess family needs and engage with young person and family
\
3. Address psycho-social needs of the family and mediate if required
\
4. Provide mental health care to young people including engagement, risk assessments, case management, support and liaison with family and other networks
\
5. Demonstrated cultural competency, particularly in working with Indigenous people
\\/ul\
Requirements:
- Relevant formal qualifications in Social Work or a related field
- A working history within the Youth Mental Health Sector and/or lived experience will be highly regarded.
- Strong interpersonal and communication skills including report writing, case note writing, case coordination, facilitating meeting and education sessions.
