About the Role

We are seeking two dedicated Aftercare Officers to join our Culture Care Connect (CCC) program, one based in Midland and the other in Parmelia. The Culture Care Connect (CCC) program will bring together key streams of suicide prevention planning, coordination and activity across the network regions, including raising awareness, early intervention, crisis management and aftercare services.

Moorditj Koort recognises Aboriginality as a genuine qualification for this position under 50D of the Equal Opportunity Act 1984. To apply you must be of Aboriginal or Torres Strait Islander (ATSI) descent, identify as ATSI and be recognised as such by the ATSI community.

What You'll Do
* Provide culturally safe aftercare, follow-up and casework support
* Walk alongside families to reduce risk and support recovery
* Build strong referral networks across health, mental health and community services
* Work collaboratively with counsellors, psychologists and SEWB teams
* Support community awareness, education and engagement activities
* Maintain accurate case notes, data and reporting
* Contribute to co-designed, community-led suicide prevention approaches
* Support families with complex social and wellbeing needs
What You'll BringEssential
* Certificate IV in Mental Health (mandatory)
* Aboriginal and/or Torres Strait Islander person (Section 50D requirement)
* Experience in case management or client support in social and emotional wellbeing or mental health
* Strong cultural knowledge, connection, and understanding of Aboriginal family systems
* Ability to yarn, build trust, and engage respectfully with clients and families
* Experience working with people with complex social, emotional or wellbeing needs
* Strong communication and relationship-building skills
* Ability to work independently and within a multidisciplinary team
* Good time management and organisational skills
* Competence with record keeping, reporting and data entry
* Current WA Driver's Licence
* WWCC, National Police Clearance (within 3 months), First Aid Certificate
* COVID-19 vaccination evidence
Desirable
* Knowledge of local community and cultural protocols
* Experience delivering community education or wellbeing programs
* Experience working in suicide prevention, crisis support or SEWB
* Strong computer skills (Microsoft Suite, CRM systems)
