Program Details
We are seeking a skilled and compassionate Practitioner to join our team in delivering the Staying On Track Program.
The program aims to improve community safety and reduce reoffending by providing tailored support to young people released from detention.
Our team works in partnership with the Department of Youth Justice to ensure every young person has a plan focused on reducing the risk of reoffending.
Key Responsibilities:
* Deliver client-focused support through therapeutic interventions and case management.
* Provide trauma-informed and culturally responsive case management for young people and their families.
* Support clients to understand court orders and case plans.
* Conduct safety planning and risk assessments, and develop mitigation strategies.
* Maintain accurate case notes and reports.
Requirements:
* Degree in Social Work, Human Services, Behavioral Science, Psychology, or similar.
* Current Blue Card (Working with Children Check).
* Queensland Driver License.
* Willingness to travel and work flexible hours.

* Strong understanding of relevant legislation and practice frameworks.
* Well-developed interpersonal and relationship-building skills.
