Your role in this position is to provide clinical social work services to clients/patients using evidence-based clinical practice skills. This includes providing support and intervention using counselling, education, community liaison and referrals.
* Deliver high-quality social work services to patients in a hospital setting.
* PARTICIPATE in multidisciplinary approaches for patient care, including contributing to the evaluation of Social Work services.
* BUILD health literacy and promote self-management through patient-centred care.
* Demonstrate reflective practice and a commitment to ongoing professional development.
To be successful in this role, you will require a relevant tertiary qualification in Social Work and membership of the Australian Association of Social Workers. You will need to possess strong skills in social work assessment, case management and discharge planning.
