Program Coordinator Opportunity in Hamilton
We are seeking an experienced and dedicated Program Coordinator to join our team in Hamilton. As a key member of our multi-disciplinary team, you will play a vital role in supporting young people aged 12-25 who are facing mental health challenges.
The YES Navigator program involves delivering early intervention and case management services, ensuring that young people access the right support at the right time. This includes coordinating outreach, group work, family programs, and peer support, as well as linking young people with community services and other resources.
The successful candidate will have a strong understanding of the importance of culturally inclusive and youth-friendly care, and will be committed to building strong referral pathways to support ongoing recovery.
About the Role
* Deliver assessment and short-to-medium term multidisciplinary interventions to support young people's needs.
* Support access to services for holistic needs, including general healthcare, alcohol and other drug services, and education and employment.
* Develop and review wellness plans with young people and their families to ensure safe and coordinated transition of care.
Required Skills and Qualifications
* Demonstrated experience in evaluating and improving early intervention models responsive to young people from diverse backgrounds.
* Knowledge and links with a range of youth-related community supports and networks.
* Skills and experience in managing young people at risk, including suicide and risk to others and from others.
