Job Summary
This is a unique opportunity to work as a Culturally Responsive Outreach Coordinator, supporting Aboriginal and Torres Strait Islander people experiencing homelessness, overcrowding, chronic health conditions, and complex social needs.
The successful candidate will be responsible for designing, leading, and evaluating culturally safe, trauma-informed services for people with complex needs. This will involve building trusted relationships, advocating for systemic change, and engaging in community awareness and capacity-building activities.
Key Responsibilities:
* Support outreach and case management for Aboriginal and Torres Strait Islander people experiencing homelessness, overcrowding, chronic health conditions, and complex social needs.
* Contribute to service delivery that is culturally responsive, trauma-informed, and strengths-based.
* Engage in community awareness and capacity-building activities and build trusted relationships to advocate for systemic change.
* Collaborate with multidisciplinary teams and foster collaborative approaches across agencies and community settings.
Qualifications and Experience:
* Relevant tertiary qualification in Social Work, Psychology, Counselling, Nursing, Aboriginal and/or Torres Strait Islander Primary Health Care, Alcohol and Other Drugs, Community Development, or related discipline; or Certificate IV (or higher) as an Aboriginal and/or Torres Strait Islander Health Practitioner with substantial experience in case management, service coordination, or program leadership.
* Demonstrated experience in community development, program coordination, and service delivery in health or community service settings, preferably within Aboriginal and Torres Strait Islander communities.
