* To provide person-centred support and therapeutic interventions empowering clients throughout their recovery journey.
This role involves delivering high-quality case management facilitating group programs providing comprehensive alcohol and other drug treatment to support long-term wellbeing stability. Work within a collaborative team that is passionate about positive outcomes for individuals communities.
Responsibilities:
• Develop manage individual case plans using evidence-based approaches
• Provide psychosocial support conflict resolution facilitate group work
• Collaborate colleagues achieve sustainable client outcomes
• Maintain accurate records participate in reviews conduct screenings engage families external stakeholders contribute continuous quality improvement WHS requirements staff mentoring
