The role

The Clinical Coordinator will work 4 days per week from our Dandenong South office as part of a community-based team, supporting infants, young children and their families to achieve optimal safety, connection, development and wellbeing goals.

The Clinical Coordinator works competently within their scope of practice to deliver and lead safe, evidence informed, person-centered care to achieve optimal outcomes for all children, providing face to face support to families in their own homes.

The role also involves reviewing referrals for eligibility, pre-admission interviews, discharge planning, referrals to ongoing support where required and report writing requirements. As a leader, this role will involve making decisions regarding case direction and risk management in line with current legislation and QEC policy and procedures.

Reporting to the Program Coordinator, the Clinical Coordinator is recognised as an integral member of the multidisciplinary team across various programs, working in alignment with QEC's Model of Care, Clinical Practice Framework and the Victorian Government Best Interests Case Practice Model.

About you

To be considered for this role you will need to demonstrate:

* Tertiary qualification in Social Work, Psychology (or equivalent) or Division 1 Registered Nurse/Midwife, OR
* for a candidate with Diploma level qualification and extensive industry experience.
* Demonstrated leadership experience in supervising and/or coaching staff or willingness to learn and develop this skill
* Demonstrated experience working with families experiencing multiple vulnerabilities
* Theoretical understanding of child and family health, wellbeing, safety and development.
* Experience in implementing an evidence informed approach to promote positive outcomes for children and families
* Understanding of the legislative requirements and guidelines that underpin Child and Family service system

The successful candidate will have access to QEC's Clinical Starter Program which has been designed to support all new clinicians in developing skills and building knowledge to support them in the role of Clinical Coordinator. This program offers new starters ongoing support which includes one on one support from our learning team, a QEC program handbook, extensive induction and orientation program as well as scheduled clinical training.
