Disability Support Worker Job Description
We are seeking experienced and resilient Disability Support Workers to provide high-level, person-centred support to individuals with complex needs.

6. Work collaboratively with multidisciplinary teams to develop and implement individualised plans, ensuring seamless transitions between care settings.
7. Support clients with justice or correctional involvement, including adhering to behavioural plans, risk management strategies, and supervision requirements.
8. Promote community participation while maintaining safety for clients, staff, and the public.
9. Assist clients to meet the expectations of court orders, probation orders, or behaviour management plans (as applicable to disability support).
10. Maintain professional boundaries and clear, factual documentation needed for justice-linked reporting.

Requirements:
* A minimum of 1-2 years' experience working with individuals with disabilities or mental health conditions.
* Relevant qualifications in disability support, social work, or a related field.
* Excellent communication and interpersonal skills, with ability to build strong relationships with clients, families, and stakeholders.
* Ability to work independently and as part of a team, with a strong focus on collaboration and mutual respect.
* High level of empathy and understanding of the complexities of disability support.
* Familiarity with relevant legislation, policies, and procedures, including NDIS and justice-related frameworks.
