We are seeking an experienced Registered Nurse to join our dedicated team as a Health Liaison Officer supporting Cootharinga and CPL clients across Townsville and surrounds. We are a leading community support provider across Queensland and Northern Recent South Wales, partnering with people with disability and their families to create meaningful change.

With preferred experience in disability and / or community, you will use your nursing skills to provide health advice, facilitate health support education to support workers and / or clients, and liaise with external health professionals, assisting clients to navigate their health journey and support needs. Based in Townsville, with an opportunity for occasional travel, the position will support Cootharinga and CPL teams to provide assistance for people with disabilities.

Responsibilities

- Developing and reviewing individual health support plans for people with complex disability and health care needs in consultation with the individual, their medical practitioner and other relevant health care professionals.
- Deliver training to provide theoretical and competency-based education to Cootharinga and CPL support workers in class, online and individualised training for specific support needs; assisting with the review / creation of educational material when required.
- Ensure medication and health administration practices are in line with Cootharinga and CPL’s scope of practice, policy and procedures, professional guidelines and quality standards.
- Attend specialist and complex health appointments when required.
- Provide clinical advice, resources, information and expertise to individuals and their service delivery teams.
- Liaise with hospital discharge teams, allied health professionals, support coordinators, and service delivery teams to ensure safe transition through significant health events.
- Champion person-centred care practices, by ensuring that clients are offered the chance and encouraged in a positive manner to make their own decisions and choices about their lives and lifestyle.
- Support clients with informed health-related decision-making by ensuring that clients receive relevant information in relation to their individual circumstances.
- Collaborate with the wider Health Liaison team to ensure medication and health procedures / guidelines and support plans are reflective of current best practice and can be easily understood by service delivery teams, clients and families.

Qualifications

- Current AHPRA registration with minimum 2 years' post-qualification experience.
- A strong clinical background with experience in complex care planning, preferably in a community setting.
- Proven experience in disability and / or assisting a person with complex health needs.
- Training experience preferred, but not essential.
- Excellent communication and interpersonal skills.
- Good technology skills.
- Strong problem-solving and time management abilities.
- Opportunity to travel to other regions intermittently (planned in advance).
- Ability to work autonomously.
- A current driver's license.
- NDIS Worker Screening Clearance (We can help!).
