About the Role
As a Placement Services Welfare Officer, you support patients and families to navigate aged care options and safe community living. You build strong connections, maintain vital service knowledge and work closely with staff to ensure smooth, timely placements.
Key duties
* Assess patient needs and broker access to aged care services
* Maintain up-to-date knowledge of aged care facilities and options
* Liaise with patients, families and external service providers
* Support clinical staff with residential and community placements
* Monitor and record placement outcomes to ensure continuity of care
About You
You are a proactive and organised professional who thrives helping patients and families navigate aged care options. You communicate clearly, work independently, and build strong relationships across teams and services.
* Use initiative and work independently with minimal direction
* Advocate effectively for patients across services and agencies
* Deliver high-level customer service to diverse client groups
* Maintain accurate records and use Word, Excel and other systems
* Apply knowledge of health, welfare, confidentiality and safety
