Home Care Package Leader

We are seeking an experienced and skilled Home Care Package Leader to coordinate and oversee the delivery of high-quality, client-centered care. In this leadership role, you will work collaboratively with the Case Management team and other key stakeholders to ensure that services are provided effectively, efficiently, and in alignment with the client's goals.

* Lead and manage the Home Care Package Program, ensuring effective coordination of services and resources.
* Develop and implement strategies to improve service quality, client outcomes, and program efficiency.
* Collaborate with senior management and other teams to identify and address any operational or logistical challenges.
* Contribute to marketing efforts to promote the Home Care Package Program and drive growth.
* Identify opportunities for process improvements and implement changes to enhance care quality and competitiveness.

Requirements:

* Minimum 3 years' experience in aged care, home care, or a related field.
* Sound knowledge of the Home Care and/or Aged Care industry, including relevant standards, regulations, and accreditation processes.
* Strong leadership and communication skills, with the ability to build rapport with internal and external stakeholders.
* Experience in budgeting and resource management.
