What You'll Be Doing
Providing Enhanced ACcess to Health services (PEACH) program is an innovative project established at the Sydney Children's Hospitals Network (SCHN). The aim of this project is to develop sustainable pathways and technological solutions that enhance access to health services for children and young people from priority populations. The purpose is to address health inequities and improve access to appropriate health services and programs. The target groups include children and young people who are culturally and linguistically diverse (migrants, refugees and asylum seekers); identify as Aboriginal or Torres Strait Islander, live in out of home care (OOHC); live with a disability or are at high risk of a disability; socioeconomically disadvantaged; identify as other priority populations.
The Priority Populations Care Navigators (PPCNs) will work across SCHN and with the PEACH Working Group and Steering Committee to facilitate implementation of PEACH. The key focus of the PPCNs is to develop and implement new SCHN-wide systems and navigation pathways to identify patients from priority populations and to deliver enhanced healthcare access, utilising technology to assist where possible.
The PPCNs will undertake project management tasks, including monitoring and reporting progress against Key Performance Indicators (KPIs) and building capacity and driving cultural change through engaging staff and key stakeholders including families and carers. These steps will be critical to ensuring sustainability of the PEACH Project beyond the funding.
The program involves collaboration with Local Health Districts and other external partners, as well as SCHN treating teams and departments such as SCHN Diversity Health, Refugee Health, NDIS Transition Coordinators and Aboriginal Health. This position sits within Priority Populations, a clinical program within the Sydney Children's Hospitals Network (SCHN).
Please note that throughout the recruitment process you will be assessed with the following:
* Tertiary qualification in a health care discipline and /or equivalent experience.
* Demonstrated experience in project management, change management and quality improvement in a health setting.
* Highly developed interpersonal and communication skills and demonstrated high level written and presentation skills.
* Demonstrated ability to be self-directed and prioritise work within an environment of competing priorities to meet deadlines with minimal supervision.
* Demonstrated understanding of how diverse backgrounds and vulnerable populations can impact on access to, and experience of health and health care systems.
* Demonstrated high level influencing, negotiation and facilitation skills and commitment to delivering organisational change, including demonstrated ability to develop and maintain effective working relationships with clinical staff and other key stakeholders.
* Demonstrated computer literacy, I.T skills and proficiency to use same to support quality improvement and project management initiatives.
* This is a position identified for Aboriginal or Torres Strait Islander people on the basis of a genuine occupational qualification under section 14(d) of the Anti-Discrimination Act 1977.
