The Position
* Our innovative service model focuses on delivering the best clinical outcomes for clients. As a District Nurse with Empower, you will be responsible for the delivery of Empower Healthcare's comprehensive services across home care clients in Geelong. You will:
* Conduct comprehensive clinical assessments and develop clinically appropriate care plans
* Provide a range of nursing services to clients within the community, including but not limited to Medication Management, Wound Care, Diabetes Management, Catheter Care, Stomal Therapy and Cognitive Assessments
* Work holistically with other healthcare professionals to provide a multidisciplinary approach to client care
* Educate clients and their families to optimise the client's ability to live safely within the community
* Work collaboratively with Case Managers and NDIS Support Coordinators to provide positive client outcomes
* Work autonomously to prioritise and complete tasks and complete all required documentation to a quality standard.

* Unrestricted Registration with AHPRA as a Registered Nurse
* Full time availability Monday to Friday
* Sound clinical knowledge and skills
* Experience working with clients who have chronic and complex health needs
* Experience working with clients from diverse cultural and socio-economic backgrounds
* Excellent verbal and written communication skills
* Excellent time-management skills and attention to detail
* National Police Check Clearance
* NDIS worker screening
* Availability on Mondays and Fridays
