Job Title: Dementia Support Coach
We are seeking a highly skilled and compassionate Dementia Support Coach to join our team. As a Dementia Support Coach, you will play a critical role in providing on-the-ground support to individuals living with dementia and their caregivers.
Your primary responsibility will be to facilitate the implementation of relationship-based support strategies, modelling and coaching clients, carers and care staff. You will work collaboratively with peers to develop the skills, expertise and applied knowledge of aged care staff and carers to understand the person living with dementia and deliver best practice support and care.
* You will provide one-on-one coaching and guidance to clients, carers and care staff to enhance their understanding of dementia and improve their ability to provide high-quality care.
* You will participate in the development of care plans that meet the unique needs and goals of each client.
To succeed in this role, you must possess strong organisational abilities, including effective time management and prioritisation skills. You should also have excellent communication and interpersonal skills, as well as a demonstrated commitment to delivering exceptional customer service.

Requirements:
* Qualification in a relevant field (e.g. Allied Health Assistant, Diversional Therapy, Enrolled Nursing, Aged Care)
* Strong organisational abilities, including effective time management and prioritisation skills
* Excellent communication and interpersonal skills
* Demonstrated commitment to delivering exceptional customer service
* A current driver's licence and safe driving record
