The Care Coordinator supports the Care Manager in providing leadership, support and guidance to Home Care Package Partners, Lead Practitioners and Administration Officer. The Care Coordinator will oversee the Home Care Package and NDIS Service provision, consistent with the mission of Blue Care, and assist in effectively coordinating the human, financial and material resources. Please note the Care Coordinator may be portfolio or geography based contingent upon variable factors including but not limited to location of role, this will impact scope of role in supporting staff.
**Key Responsibilities**:
- Deliver consistently professional and innovative person-centred care and quality outcomes for clients in accordance with Blue Care requirements and funding models
- Provide a high standard of flexible and responsive leadership and expert clinical advice and oversight relating to complex care issues
- Ensure client care is assessed, planned and implemented and evaluated in accordance with Blue Care's Tailor-Made Service Model as well as the Departments Funding Model, taking into consideration each client's physical, social, emotional, spiritual and clinical needs and encouraging client / client representative participation.
- Assist with orientation of new staff and ongoing professional development of staff.
- Undertake other duties as required/directed as service needs change or grow (consistent with the above, within the scope of the position and any applicable Duties List and any other legal or industrial obligations).
**Knowledge and Experience**
- Extensive knowledge and experience in case management, nursing or allied health or human services with demonstrated skills reflective of functional experience where scope of role includes clinical or care staff support.
- Proven leadership capability within a multidisciplinary team.
- Demonstrated understanding and sound knowledge of client centred, community-based care services delivery, or willingness to learn quickly.
- Ability to liaise with the clinical and care team and to provide co-ordinated care.
- Demonstrated experience and knowledge in Community Funding streams and programs
- Knowledge of all relevant legislative, regulatory requirements, professional standards and guidelines relevant to the Aged Care Community Services environment with Quality Systems, Accreditation, WHS and Continuous Quality Improvement.
**What you'll need**:
Essential:
- Qualifications in a health, community service or other relevant discipline or equivalent community sector experience.
- All Blue Care staff must satisfy the relevant probity checks required by legislation or Blue Care policies.
Desirable:
- Post Graduate qualifications in a Health or Community Service discipline or Management or working towards
- Current First Aid Certificate and Cardiopulmonary Resuscitation (CPR) certificate or willingness to obtain if required.
- Knowledge and understanding relevant legislative frameworks, standards and government funding requirements and guidelines
