Your Opportunity:
* Provide High-Quality Mobile Dietetics Services to Adults in Residential Aged Care Facilities and the Local Community
As a Registered Dietitian, you will work under the guidance of your Dietetics Team Leader to deliver person-centered care to clients. Your responsibilities will include:
* Conducting Person-Centered Dietetics Assessments and Developing Personalized Nutrition Plans
* Improving Nutrition and Hydration Systems in Residential Aged Care Homes
* Delivering Educational Sessions on Healthy Eating and Nutrition
* Completing Menu and Mealtime Assessments to Ensure Optimal Nutritional Outcomes
* Collaborating with External Stakeholders and Plena Colleagues to Deliver High-Quality Dietetic Care
* Staying Up-to-Date with Contemporary Best Practice and Continuous Improvement
* Accurately and Timely Completing Clinical Documentation
* Contributing to a Positive Team Culture by Embodying Plena's Values and Engaging with Colleagues
