You will implement and evaluate engaging activities that promote a fulfilling lifestyle for clients at home and in residential care.Using a primary health care approach, you'll oversee volunteer services and organize social and diversional activities, offering individual support tailored to various cognitive and physical needs. Collaborating with a multidisciplinary team, you'll ensure the delivery of coordinated, evidence-based care. Join us in this rewarding role and make a meaningful impact in our community!Key Responsibilities:Consumer Assessment and Activity Coordination: Evaluate each consumer's ability to participate in activities and organise group activities tailored to their needs and cognitive abilities, including dementia programs with clear objectives and evaluation methods.Resource and Documentation Management: Ensure necessary equipment and personnel are available for safe activities while completing electronic documentation in collaboration with the Registered Nurse or NUM.Activity Planning and Team Collaboration: Develop, implement, and evaluate individual and group activity plans, working closely with staff to provide effective care for the Penola community.Volunteer Engagement and Program Review: Support volunteers and advocates in activity programs, provide guidance, and regularly assess programs with consumers to gather feedback for improvement.Computer Skills and Resident Care: Utilise computer skills for electronic care planning and contribute to the delivery of high-quality resident care.What We’re Looking For:Proven experience in providing direct care services within a health-related field.Experience working with aged clients and individuals with disabilities, along with their families.Background in a relevant field, demonstrating specialist knowledge and experience to advise on activities and contribute to objective-setting.Previous experience in a Diversional Therapy or Activities Coordinator role.Proficient in using computer software packages, including Microsoft Word and Excel.Experienced in implementing strategies that promote independence, good health, and optimal wellness for aged and disabled clients in a day centre setting.Why Join Us?Make a real difference and feel like an important part of the community.
