Job Description: Role Title: Senior Social Worker - PEIPOD Role Type: Permanent, full-time (Monday to Friday, 8:30am–5:00pm) Role Grade: Social Worker Level 3 Department and Location: Community Mental Health Service Salary: $116,892 $120,680 p.a. About the Role: Permanent role working with the Program for Early Intervention & Prevention of Disability (PEIPOD) at St Vincent’s Hospital, providing comprehensive mental health assessment and intervention aligned with early intervention principles to prevent relapse and longer-term impairment. Work as a primary clinician to coordinate care for young people, as well as providing social work–specific intervention and consultation. Provide individual and group psychoeducation to consumers and carers on mental health and AOD conditions, treatment options, harm minimisation, relapse prevention, and health promotion. Facilitate care pathways that provide seamless referral between different departments and service providers, ensuring the delivery of holistic care through the utilisation of mainstream health and welfare services aligned with St Vincent’s Hospital’s mission and policies. Support medication monitoring and side-effect management, assisting clients to develop effective strategies for medication adherence and self-management. Work collaboratively within multidisciplinary teams, maintaining regular communication with internal and external clinicians, and ensuring accurate clinical documentation, case reviews, and completion of mandatory outcome measures in line with MH-OAT and service requirements. About You: Qualified Social Worker with a degree in Social Work, eligible for membership with the Australian Association of Social Workers (AASW) with evidence of clinical experience in tertiary hospital or health related fields. Experienced clinician with extensive knowledge working within a multi-disciplinary team in an inpatient or community mental health service Demonstrated understanding of early intervention principles in the context of emerging mental health problems in young people. Experience and commitment to working with young people with complex needs, including homelessness, drug and alcohol use and forensic issue. Excellent interpersonal, communication, negotiation, and organisational skills, with a commitment to collaborative team practice, professional development, and mentoring junior staff and students through supervision and education.
