We are seeking a skilled Early Childhood Coordinator to support the delivery of our early childhood approach at a local level. This role is responsible for working closely with families to help children reach their full potential through effective planning and support, while also fostering meaningful connections within the community.
Key Responsibilities:
* Supporting families through a child and family-centred approach, ensuring children receive the appropriate interventions based on best practice evidence.
* Utilising qualifications in Early Childhood, Allied Health, Social Work or a related field to support evidence-based decision-making.
* Completing functional assessments for children with developmental concerns or disabilities, determining their needs and ensuring timely referrals to appropriate support pathways.
* Connecting families to community services and mainstream programs to enhance their social participation and access to essential services.
* Guiding families through NDIS plan reassessments, gathering evidence, assisting with goal development and ensuring the right support is in place.
* Utilising the NDIS plan to support children and their families with improved choice, control, social participation and overall quality of life.
