Community Physiotherapist
Job Description:
We are seeking a highly motivated and passionate physiotherapist to join our dynamic community-based team. This role will provide an opportunity to work in a variety of settings and caseloads, supporting the delivery of high-quality care to individuals in need.

As a Community Physiotherapist, you will be responsible for delivering evidence-based assessments, recommendations, and active interventions across a range of programs, including:
* Hospital substitution programs (Rehab at Home, Hospital in the home) - Acute and sub-acute rehabilitation in the comfort of customers' homes;
* Government Aged Care programs (Home Care Package, Short Term Restorative Care, Commonwealth Home Support Programs) - Supporting our ageing population to stay well and independent at home;
* Working independently in the community and collaborating closely with our multi-disciplinary team for optimal patient outcomes;
* Engaging in clinical supervision with a designated supervisor.
Key Responsibilities:
* To deliver high-quality patient care through assessment, diagnosis, and intervention;
* To work collaboratively with other healthcare professionals to achieve optimal patient outcomes;
* To maintain accurate records and communicate effectively with patients, families, and healthcare teams;
* To participate in ongoing professional development to stay current with best practices and research.

Requirements:
* Possession of a Physiotherapy degree with unrestricted AHPRA registration;
* Valid driver's licence, insured roadworthy vehicle;
* Previous experience working in community or hospital settings preferred.
