About Greenway Views

LDK Seniors’ Living’s flagship village, Greenway Views, is in Greenway, Tuggeranong in Canberra, opposite the South Point shopping centre and walking distance to the Tuggeranong town centre. The village consists of 327 apartments, spread over five buildings that is about half a kilometre long from end-to-end. Greenway Views is home to nearly 400 residents over the age of 65. The village has over 6,000 square metres of communal space facilitating social connectedness and a vibrant village community. LDK delivers lifestyle-for-life through a variety of age-friendly facilities, amenities and activities for both independent and non-independent residents. On-site facilities include a gym, art centre, chapel, dance room, theatre, library, several hospitality outlets, village shed, barber shop, hairdresser, medical consultation room, grocer, purpose designed meeting spaces and a dog park.

About the Role

Reporting to the Clinical Case Manager Lead, the Clinical Case Manager supports integrated and coordinated resident care and service requirements, including pre-entry clinical assessment, move-in support, care planning, care delivery, care evaluation, referral and accessing externally funded care (with associated documentation), coordination of support for residents and networking.

Role Responsibilities:

The Clinical Case Manager has an important leadership role within the care team and liaises with a wide range of internal and external stakeholders. The position works Monday to Friday 0830am - 5pm with competitive rates in the industry plus superannuation. Duties include but are not limited to:

* Streamlining care for your portfolio of residents
* Conducting timely and comprehensive clinical assessments
* Overseeing resident support to determine precise care needs
* Facilitating smooth transitions to LDK Homecare AP where needed
* Welcoming new residents to the Village, introducing them to the care team and assistive technologies
* Assisting residents in accessing externally funded care
* Coordinating aged care assessments through ACAT/MyAgedCare
* Navigating Commonwealth Home Care Packages and Support Programs
* Handling Veterans' care coordination, including GP communication and documentation
* Assessing eligibility for external care services
* Collaborating with the Clinical Case Manager Lead to meet resident goals and preferences

Qualifications/Certificates/Professional Experience

* AHPRA registration
* Bachelor of Nursing
* Experience in the community care sector/with home care packages
* Aged care nursing experience desirable
* Experience working as a member of a multidisciplinary team
