About the role
The Care Support Partner is primarily an office-based role, working collaboratively with participants, key stakeholders and Care Partners through a team-based approach, ensuring that participants needs and requests are actioned. Providing comprehensive care planning, individualised case management, wellbeing and support to participants, this role promotes wellness outcomes.
What will this look like on a day-to-day basis?
You will be:
* Supporting Care Partners to ensure all assessments, reviews and other documents are current, documented and reviewed as needed
* Providing phone support to participant enquiries and feedback
* Working collaboratively and coordinating with other team members, community organisations, allied health providers and service agencies
* Recording and maintaining high quality assessment and care planning documentation, case notes and records of conversation
* Providing comprehensive case management, ensuring the day-to-day wellbeing of our participants through case work, advocacy and planning
* Fostering respectful relationships with participants to fully understand their needs and ensure our services are reflective of person-centred practice and the Aged Care Quality Standards
* Supporting Care Partners to manage participants budgets, ensuring appropriate use of funding in line with Support at Home program
* Monitoring and responding to changing participant needs and engaging in ongoing conversations with participants and other key stakeholders to support wellness and reablement outcomes
* Supporting the participant by providing independent advice and information
* Managing participant feedback and complaints regarding service delivery in line with internal policies and ACQS requirements, undertaking remedy actions to address issues
What skills and experience do you need to be a Care Support Partner?
This position is best suited to someone who:
1. Minimum Diploma qualification in Nursing, Community Services (Case Management), Human Services, Health Sector Management, Aged Care or Disability and a minimum of 3 years' experience in the industry
2. Has a demonstrated understanding of the Aged Care reform, Strengthened Aged Care Standards, the sector and people who are aging in the community
3. Is experienced in case management preferably within the home and community care sector with advanced knowledge of person-centred care
4. Is relatable and engaging with highly developed interpersonal skills, communication skills and emotional intelligence
5. Understanding of specialized service delivery within diverse groups, particularly in relation to Aboriginal and Torres Strait Islander, CALD, Veterans, Disability, Mental Health, LGBTI+, and Homelessness
