About the Role:
We are seeking a seasoned leader to oversee our multidisciplinary team responsible for delivering high-quality, person-centered care that reflects our model of care.
The ideal candidate will have substantial experience leading teams within care coordination, case management, or allied health settings and possess strong leadership, communication, and analytical skills with a commitment to transparency and accountability.
Key Responsibilities:
* Leading a multidisciplinary team to deliver high-quality, person-centered care that reflects our model of care
* Implementing Continuous Quality Improvement (CQI) practices, including audits, incident review, and improvement planning
* Monitoring billable hours and cost recovery, with financial literacy and expertise in data-driven reporting
Requirements:
* Tertiary qualification in Social Work, Psychology, Community Services, or a related field
* Strong understanding of the Strengthened Aged Care Quality Standards and NDIS Practice Standards
* Current Working with Children Check, National Police Clearance, and Driver's Licence
