Our Intensive Case Management Services (ICMS) support young people who are assessed as high risk and are involved with the child protection or statutory systems. These young people experience multiple, complex behavioural and emotional challenges and require long‐term, intensive, and consistent support.
ICMS provides intensive, relationship‐based case management for young people for whom conventional or less intensive approaches have not been effective. Our work is proactive, trauma‐informed, and grounded in strong partnerships with young people, families, and key stakeholders.
About the role
As a Case Manager – ICMS, you will provide intensive, wrap‐around support to young people through a high‐contact, outreach‐based model. You will:
* Build strong, purposeful relationships with young people and their families
* Deliver individualised, strengths‐based case management
* Actively involve young people in assessment, planning, and decision‐making
* Coordinate multidisciplinary supports and advocate within statutory systems
* Respond flexibly and assertively to changing risks and needs
* Work within a collaborative, trauma‐informed team environment

About you
You will bring:
* A relevant tertiary qualification (e.g. Social Work or equivalent)
* At least one year's experience supporting young people with complex needs and high‐risk or challenging behaviours
* A strong understanding of the developmental, emotional, and therapeutic needs of young people who have experienced trauma, abuse, and neglect
* Knowledge of a range of theoretical frameworks and the ability to apply them in work with young people on statutory orders
* Demonstrated ability to work within a case management framework, including assessment, planning, review, and adaptation of interventions
* Highly developed communication, organisational, and time‐management skills
