A Clinical Pharmacy Professional with a strong track record of delivering high-quality patient care and expertise in medication management. Key responsibilities include:
* Dispensing and supplying medications and pharmacy products
* Coordinating patient medication requirements for inpatient stays and discharges
* Conducting medication chart reviews and reconciliation
* Developing and maintaining professional relationships with key stakeholders to ensure excellent customer service to clients and patients
* Supporting patients, carers, hospital staff and associated employees
* Educating and advising staff and customers on health promotion, disease prevention, drug selection and the proper use of medicines
Role Overview
As a Clinical Pharmacy Professional at our pharmacy, you will be responsible for delivering clinical pharmacy services across both inpatient and discharge settings.
