Role Overview
The Care Coordinator Specialist plays a vital role in ensuring seamless patient care across the endoscopy referral journey. Key responsibilities include reviewing referrals, performing assessments, and providing support to patients in navigating necessary documentation for care.
Main Responsibilities:
* Performing triage and referral assessments according to established guidelines.
* Escalating complex cases to clinicians for timely intervention.
* Adding patients to wait lists, coordinating readiness for care, and employing a patient-centred approach in all interactions.
Requirements:
To be successful in this role, you will require a Registered Nurse with current AHPRA registration, experience in senior clinical roles (CNS or above), and completed or working towards postgraduate studies in your area of speciality. Knowledge of current trends affecting public health is also desirable.
