We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.About the RoleAs a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Canberra and surrounding areas.The successful candidate will be responsible for processing referrals, assisting clients in commencing services, and continuously reviewing and adjusting care plans to suit their evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.This is a permanent full-time role, based in our Griffith office with hybrid work from home arrangements and travel to clients homes surrounding suburbs when required.Key ResponsibilitiesUndertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholdersEnsure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visitsCollaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering team and Practice LeadsDevelop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the communitySkills & ExperienceDemonstrated Aged Care or Nursing case management experienceUnderstanding of Home Care Packages, Commonwealth Home Support Programme, Aged Care services and My Aged Care StandardsAbility to effectively build relationships with individuals, families and/or stakeholdersStrong administration skills and competency in utilising a variety of technologyProficiency with Microsoft Office, Outlook and database systemsAbility to identify and implement risk mitigation strategiesHigh level communication, influencing, liaison and negotiation skillsEstablished problem solving and conflict management skillsAbility to multi-task and work autonomously in a fast-paced environmentA current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travelCareer pathways across a national organisationAttractive remuneration package with a monthly rostered day off (Think 12 extra long weekends a year!)Receive up to $15,900 of your annual salary tax-free through Salary Packaging, increasing your take-home payJoin an established organization as we move into new aged care reformsLive and work in your local communityHybrid work from home arrangementsSuccessful candidates will be required to clear probity checks including National Criminal History Record Check and a statutory declaration.How to ApplyInclude your resume and covering letter in one document, click 'Apply' and follow the prompts.
