We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.About the RoleAs a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Canberra and surrounding areas.The successful candidate will be responsible for processing referrals, assisting clients in commencing services, and continuously reviewing and adjusting care plans to suit their evolving needs.
