The Aged Care Clinical Care Partner role is an exciting opportunity to deliver person-centred, evidence-based care to older Australians in their homes.
Key Responsibilities:
1. Deliver high-quality nursing services, including wound care and medication management
2. Monitor client health, elevate concerns to healthcare professionals and contribute to clinical governance and quality improvement strategies
3. Assist with reporting, risk management, and quality improvement initiatives
4. Build trusting relationships with clients, families, and carers
5. Promote shared decision-making and collaborate with healthcare professionals to ensure holistic care
The ideal candidate will have a current AHPRA registration as a Registered Nurse, at least 3 years of experience, and demonstrated skills in care planning, case management, and clinical decision-making. Proficiency in digital systems is also essential.
